Notice to all Local 1579 Members.  A Connecticut workers comp form 30C is required whenever a first report of injury is filed, even if medical treatment was not required.  This documentation with the State is the only recognized legal report of an injury.  The submission of this report is the RESPONSIBILITY OF THE  EMPLOYEE, NOT THE DEPARTMENT.
